Abstrak

Background: Tuberculous spondylitis is an extrapulmonary tuberculosis of the spine that can cause deformity, spinal instability, and neurological deficits. Data on patient profiles at Prof. Dr. I.G.N.G. Ngoerah General Hospital are limited. This study aimed to describe demographic, radiological, hematological, and treatment characteristics of patients with tuberculous spondylitis. Methods: A descriptive cross-sectional study was conducted using medical record data of patients treated from 2022 to 2024 with total sampling. Variables analyzed included demographic characteristics, radiological findings, hematological laboratory results, treatment approach, and histopathological findings. Data were presented descriptively as frequencies and percentages. Results: Fifty-three patients were included with a mean age of 36.79±16.08 years, most were 21-40 years old. Males accounted for 52.8%. Active pulmonary TB was found in 24.5%. Neurological status was mainly AIS E (62.3%) followed by AIS D (28.3%). All patients had radiological features consistent with tuberculous spondylitis, most commonly in the lumbar (39.6%) and thoracic (33.9%) regions. Paradiscal lesions were predominant (81.1%), with gibbus deformity in 50.9% and skip lesions in 3.8%. Leukocyte counts were normal in 66%, while ESR and CRP were elevated in 96.2% and 73.6%, respectively. Operative management was performed in 64.2% of cases and typical tuberculous histopathology was confirmed in 47.1% of biopsied patients. Conclusions: Patients with tuberculous spondylitis at Prof. Dr. I.G.N.G. Ngoerah General Hospital during 2022-2024 were predominantly of productive age, with paradiscal lesions mainly in the lumbar-thoracic regions, elevated ESR and CRP as the most common inflammatory markers, and operative treatment in most cases.
